The Role

Supporting the assessment, planning, delivery, and evaluation of personalised health and therapy care.
Working within national guidelines and internal policies to uphold best practices in care delivery.
Collaborating with nurses, therapists, and wider clinical teams to ensure smooth and effective care.
Promoting patient choice, dignity, and independence in all aspects of care.
Supporting junior team members, volunteers, and trainee Assistant Practitioners.
Helping to create a culture where safe, respectful, and compassionate care is prioritised.

About You

Level 5 Foundation Degree in Assistant Practitioner (Healthcare) – essential
Experience working in a healthcare setting (preferably within neuro/palliative/specialist services)
Strong communication and interpersonal skills
Ability to build relationships with patients, carers, and professionals
Knowledge of healthcare legislation and safe practice guidelines
Experience working in a multi-disciplinary environment
Ability to respond empathetically to people in distress
